I'm not having trouble with any other type of learning object.  But when I try to add a question to a survey I get the following error: Security issue, the request seem invalid ! Try a new login and retry. [logout]

Any ideas?

I forgot to mention I'm in version 3.6.0.4

you should apply them from the oldest to the newest.
